The following Terms and Conditions (the “Agreement”) govern your access to and use of this website, therapydave.com, and any of its services (collectively referred to as the “Site”). This Site is owned and operated by DTSC, LLC, an Oregon limited liability company, doing business as (“DBA”) TherapyDave (the “Company”), located at 2440 Willamette Street, Suite 101-C, Eugene, Oregon, 97405. When the terms “we,” “us,” “our,” or similar are used in this Agreement, they refer to the Company. By accessing or using the Site, you agree to comply with and be bound by this Agreement. Please read this Agreement carefully. If you do not agree to any part of this Agreement, you must not use the Site. Additional definitions, terms, and conditions can be found in the Privacy Policy and Disclaimer, which are hereby expressly incorporated into this document by reference.
Appointment Requests
Submitting an appointment request does not guarantee a confirmed appointment or establish a therapist-client relationship. We will review your request and contact you to confirm availability and suitability. All appointments are subject to availability, and we reserve the right to refuse service to anyone at our discretion.
Confidentiality
We are committed to maintaining the confidentiality of all communications and information shared during the therapy process. However, please be aware that electronic communications, including email and web forms, may not be entirely secure. For confidential matters, we recommend using more secure methods of communication such as our secure client portal for established clients.
This site is not directed to children under the age of 18, and we do not knowingly collect information from anyone under 18 years old.
Service Availability
Therapy services are provided exclusively to clients physically located in Oregon and Arizona, where our licensed therapist is authorized to practice. For clients in other locations, we offer professional life coaching services, which are separate from therapy and not a substitute for mental health treatment. The distinctions between therapy and coaching will be explained during your initial consultation.
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under federal law, health care providers must give clients who don’t have insurance, or who are not using insurance, an estimate of the bill for medical items and services.
- Your initial New Client Appointment ($240, prepaid) is a flat fee and does not require a Good Faith Estimate.
- Ongoing therapy services may be subject to GFE rules.

Class Action Waiver
Any proceedings to resolve or litigate any dispute will be conducted solely on an individual basis, and you shall not seek to have any dispute heard as a class action, a representative action, a collective action, a private attorney-general action, or in any proceeding in which you act or propose to act in a representative capacity.
Governing Law; Forum
Any dispute between the parties regarding the subject matter of these Terms will be governed by these Terms and the laws of the State of Oregon and applicable United States law, without giving effect to any conflict of laws principles. You agree that any action at law or in equity shall be filed only in the state and federal courts located in Lane County, Oregon (except for small claims disputes, which may be filed in the jurisdiction in which you reside). You hereby irrevocably and unconditionally consent and submit to the exclusive jurisdiction of such courts.
Indemnification
You agree to defend, indemnify, and hold the Company harmless, as well as its independent contractors, service providers, and consultants, and our and their respective directors, officers, employees, and agents (collectively, the “Company Parties”) from and against any claims, damages, costs, liabilities, and expenses (including, but not limited to, reasonable attorneys’ fees).
